Wound care includes conducting a comprehensive assessment of the wound and the person and implementing care that provides an appropriate environment to promote wound healing.   

Fundamental principles include cleaning the wound of debris and protecting it against contaminants to avoid infection, application of an appropriate dressing that reflects the wound assessment, and ongoing assessment and referral on for further specialist care if needed

The role involves initial holistic person and wound assessment and creating a care plan to treat the wound effectively to promote wound healing (if healing is the goal of care), – this may include consultation with other specialist practitioners – and providing ongoing wound care education.

Are some people more susceptible to chronic wounds than others?

There are certainly people more likely to experience wounds than others, some of which may become chronic wounds if they fail to heal within an expected timeframe.

Increasing age, decreased mobility and medical conditions such as diabetes, obesity, cardiovascular disease, renal disease and lymphoedema can all contribute to wound chronicity.

Your wound care nurse will provide you with information about wound treatments, dressings, medication, skin care, and activities to avoid or undertake to support wound healing to help you make decisions about your wound care.

Other ways to promote wound healing include staying hydrated, eating a balanced diet and getting plenty of sleep. Individuals with other chronic conditions will be encouraged to follow their wound care plan.
