Chronic diseases are diagnosed long lasting conditions with ongoing effects which impact on the physical, emotional and mental wellbeing of individuals. Chronic diseases may also affect daily activities, relationships and one’s self-image.

Common chronic diseases include: arthritis, asthma, chrons disease, diabetes, epilepsy, hypertension and there are many more.

Chronic disease management is the care coordination and self-management support to help people with chronic diseases to better manage their condition and access appropriate services in order to improve their own health outcomes, prevent complications and reduce the need for hospitalisation.

Chronic disease management at home aims to improve your mental, physical and emotional well-being by reducing the discomfort you experience from the diseases as well as reduce any unnecessary hospitalisation associated with these conditions.

At Vital Home Health Services, we create individualised, person-centred chronic disease management plans for our clients. We work closely with your GP and provide regular reviews of these plans to you and your treating doctors to ensure chronic disease management plan is always current and as effective as possible.
