On many occasions, a doctor, hospital discharge planner, eyecare case manager, or another health professional will refer you to a care provider, or recommend you seek support services.

If however, you haven’t been referred, but realise you or a loved one does need that extra support, then you can contact Vital directly.

Alternatively, your doctor can also help you organise your nursing and care services.

It may be the case that someone does need extra support but is still largely capable, and desires to live in the comfort of their own home with additional supports. They may not want or need to live in an assisted living facility or nursing home.

They too may be eligible for home nursing and support services.

There is an array of funding options available for nursing and care support based on the type of care you need.

For example, there’s the NDIS, the Department of Veteran Affairs, icare, Dust Disease Care, Lifetime Care, Worker’s Insurance, and a host of others.

Vital is committed to assisting you in navigating the system of care and support services available to you in order to maximise your entitlements.
